Anchor Care & Rehabilitation Center has received accreditation for 2014 as a skilled nursing facility actively performing Quality Assurance and Performance Improvement activities.

The accreditation, presented by independent accreditor Providigm, LLC, demonstrates that the facility is continually assessing patients’ quality of life with regard to concerns such as pain, dignity, respect for a patient’s choice, and quality of care problems such as whether there are enough staff to meet patient needs, weight loss, infections, rehabilitation following acute injury or illness, and prevention of readmissions to hospital.

View the SHC 2014 Year In Review

Stakeholders and Residents celebrated the retirement of Rose Eck as Director of Restorative at Anchor Care and Rehabilitation. Rose was accompanied by two friends and former stakeholders, honored her with stories and well-wishes in her retirement.

Jose Arroyo presented to Rose a pen with three diamonds to honor her dedicated service at Anchor Care and Rehabilitation. Rose, have a wonderful celebration on your upcoming trip to kick-off the next part of your journey in life.

1159341Anchor Care and Rehabilitation is proud to announce that it has hired Rev. Steven A. Rossi as its new Chaplain/Director of Spirituality.

Steven has pastored churches for over 20 years and began his chaplain career in 2006 with Market Place Chaplains USA. He also serves as a chaplain with the Emergency Response Chaplain Corps and is a Volunteer Chaplain with the Melbourne Police Department.

For those unable to attend a SHC Hall of Fame ceremony, we’re bringing the celebration to you with four sites that were selected to have their ceremonies broadcast live via our website.

On October 9, 2014, 99 Signature HealthCARE centers across the southeast and midwest will induct over 400 distinguished individuals into the Hall of Fame, in local ceremonies featuring celebrity speakers and presentations about the inductees lives and accomplishments.

For a day that involved a lot of “Crazy Hair,” we were thankful that the end of the day resulted in these beautiful Glamour Shots. Women donned their tiaras and boas, and “men of mystery” appeared to have their Glamour Shots taken, and glamorous they truly were.


Anchor Care patients and staff kicked off “National Nursing Home Week” with Crazy Hair Day! Patients and staff went all out with some creative hairstyles. We also enjoyed ice cream, posed for glamour shots, and enjoyed a lot of smiles.

Patients, family members and staff gathered recently to celebrate those who have passed on. With fond memories and treasured words of endearment, Chaplain Basha Zackavich and Activities Director Sylvester Byrd commemorated 13 patients who all touched our lives in their own unique and endearing way.

A video montage was shared, along with the song “Legacy” by Nicole Nordeman, with all in attendance as we viewed many pictures of the times we’ve shared with them here at Anchor Care & Rehabilitation Center.
