2014-QAPI-Accreditation-Seal1

Anchor Care & Rehabilitation Center has received accreditation for 2014 as a skilled nursing facility actively performing Quality Assurance and Performance Improvement activities.

The accreditation, presented by independent accreditor Providigm, LLC, demonstrates that the facility is continually assessing residents’ quality of life with regard to concerns such as pain, dignity, respect for resident choice, and quality of care problems such as whether there are enough staff to meet resident needs, weight loss, infections, rehabilitation following acute injury or illness, and prevention of readmissions to hospital.

Residents, family members and staff gathered recently to celebrate those who have passed on.
With fond memories and treasured words of endearment, Chaplain Basha Zackavich and
Activities Director Sylvester Byrd commemorated 13 residents who all touched our lives
in their own unique and endearing way.

A video montage was shared, along with the song “Legacy” by Nicole Nordeman, with all in attendance
as we viewed many pictures of the times we’ve shared with them here at Anchor Care & Rehabilitation Center.

Recently, Signature Healthcare’s COMPASSION FUND committee stepped in to help
when one of our very own stakeholders was in “dire straits.” Having been surprised by

As part of a corporate-wide endeavor, on Wednesday, Sept. 25th,
Anchor Care and Rehabilitation Center hosted their first “Senior Olympics.”

Accepting the invitation to compete, 7 residents from Life Care of Palm Bay
joined us for an afternoon of challenges, skill and a lot of laughter.
With a total of 18 resident-competitors, they each had 4-5 minutes per event
to try to get as many points they could get by accomplishing the task for each event.

Facility Champions were chosen for each facility:

Elizabeth
(Anchor Care)  and  Stephen  (Life Care)

Top 3 winners in the first 3 categories (Golf, Ring Toss and Bean Bag Toss)
with the highest points total were:

Cecelia  (Anchor Care), Sarah  (Anchor Care) and Stephen (again … Life Care)

We all had a great time and enjoyed spending the afternoon with our neighbors.
